Marital Status
While abroad, it is required by law to notify the Consulate of marriage, divorce and/or widowhood by means of an official, signed, authenticated certificate.
Requirements for updating personal status:
- Completed and signed Notification of changes in personal status
- The original certificate (Marriage/Death) or Judgement of Divorce with an Apostille authentication. Contact the state house to find out about the hours of this service:

Apostille must be issued by the same state issuing your marriage/divorce/death certificate. If your certificates were not issued in Massachusetts, New Hampshire, Maine, Vermont or Rhode Island, you have to contact the authentication office (Apostille) in the state issuing your documents.
- Both partners passports (including a non Israeli partner) or a notarized copy (if processed by mail). If the non-Israeli spouse can not arrive to the consulate, the form must be signed by him/her in the presence of a notary public prior to submiting the notification at the Consular Department.
Change of Name
An adult, over the age of 18, may change his/her last name (family name) and/or first name. The change is subject to the approval of the Ministry of Interior. One who changes his/her name may not, by law, change it again for 7 years.
Requirements for change of name:
- You must submit the request, IN PERSON, at the Consulate.
- One complete Notice form for a change/choice of name.
- Please note that the change is subject to the approval of the Ministry of Interior and your old name will remain UNTIL confirmation. Once you receive the confirmation please contact the Consular Department.
Registration of an Israeli born abroad
A child born to an Israeli citizen (including a child born outside of Israel as first generation abroad) is considered an Israeli citizen. The child must be registered as a citizen, must possess an Israeli passport when traveling to Israel, and must arrange his/her army status once sixteen and a half years old.
Requirements for Registration of an Israeli citizen born abroad:
- Completed newborn registration form.
- The original birth certificate with an Apostille authentication. Apostille authentication is issued at the state house:

- Both parents' passports
- Registration will be done at the consulate by at least one of the parents (an Israeli parent).
- If a passport for the baby is also needed, the baby must be present at the consulate
Please be sure to arrive at the consulate at least 45 minutes prior to closing time.
Population Registry
The following services pertaining to the Population Registry are available:

- Information about a passenger
- Tracing of relatives
- Certificates of non-criminal record
Documents from the population registry:
The documents available are: Birth Certificate, Death Certificate and excerpt from the population registry
- Please complete the Request for Information from the Population Registry form.
- If the request is processed by mail, it must be signed in the presence of a notary public.
- Add your address and any contact information to avoid delays. Processing time is at least 12 weeks.
- Provide a copy of your passport.
Information about a passenger:
- For information regarding entry/exit dates from Israel, please complete the Request for Information about a Passenger form.
- If the request is processed by mail, sign the form in the presence of a notary public.
- Processing time is at least 12 weeks until you receive the certificate.
- Provide a copy of your passport.
Requirements for Tracing of relatives:
- If the request is processed by mail, the application must be signed in the presence of a notary public.
- Please include a $4 money order per application.
- Please provide a self-addressed envelope and any other contact information, to avoid unnecessary delays.
- Provide a copy of your passport.
